Barcelona line. A multicentre validation study of a facial projection reference in orthognathic surgery

The Barcelona line (BL), defined as a true vertical line passing through the soft tissue nasion in natural head position, was evaluated as a reference for planning maxillary sagittal position in orthognathic surgery. In this multicentre cross-sectional study, 74 raters, including healthcare professionals and laypeople, assessed 40 Caucasian facial profiles from Spain and Brazil. Profiles with the upper incisor positioned at or slightly ahead of the BL (0–4 mm or ≥4 mm) received the highest aesthetic ratings, while retrusive profiles (≤−4 mm) were rated least attractive. A moderately strong correlation (r = 0.68) was found between upper incisor position relative to the BL and positive aesthetic perception.
